Facet Joint Pain and Radiofrequency (RF) Denervation

The facet joints are the small joints that connect the vertebrae from behind. With age they can wear down and become a source of back pain that worsens when you stand or lean backward. For this type of pain there are non-surgical options; radiofrequency denervation is one of them.

How is facet pain recognized?

Facet pain is usually mechanical back pain that worsens with standing, walking and leaning backward, and eases when sitting. Unlike a herniated disc, it usually does not radiate strongly into the leg. The diagnosis is supported by examination and, when needed, a diagnostic test block.

Conservative approach first

Physiotherapy, core-muscle strengthening, posture adjustment and medication are the first step. When pain persists despite this, interventional procedures become an option.

Radiofrequency (RF) denervation

If a test block temporarily relieves the pain, RF denervation can give longer-lasting relief: under imaging guidance, the small nerve carrying pain from the joint is 'switched off' with heat. It is an outpatient, incision-free procedure and the patient usually returns quickly to activity.

What to expect and what not

RF does not 'repair' the joint but reduces the pain signal; the effect can last months and the procedure can be repeated if pain returns. It is not a solution for every back pain — so correct patient selection is essential.
